Preparations
The screening for and monitoring of patients with cardiovascular risk factors, requires two preparations.
(An alternative approach is to contact your database administrator and ask for a query to be performed on your patient database before each monitoring cycle. This query will result in a list of all patients in the clinic who have one or more cardiovascular risk factor/s).
A. Learn how to perform three operations on your EMR (electronic medical record).
- Retrieve a list of patients within a specified age range.
- Retrieve a list of patients with a common diagnosis.
- Create a new diagnosis and insert it into the problem list.

B. Revise your patients’ problem lists.
This involves the completion of two steps.
- Retrieve a list of all your patients aged 30 and above and review their problem lists.
- All problem lists that contain one or more cardiovascular risk factor/s, must undergo a revision. The minimal requirement is that the "diagnosis" CARDIOVASCULAR RISK FACTOR/S, be added to the problem list.
- Re-arrange the patient's problem list according to a fixed order, using a uniform set of diagnoses, and only then to:
