Monitoring Your Patients With Cardiovascular Risk Factors

 

1. Retrieve a list of all your patients with the diagnosis "CARDIOVASCULAR RISK FACTORS".
2. Determine the order in which these patient’s files will be reviewed. (See Order of Review; Demo).

                                                             ***
An alternative approach to steps 1 and 2 , is to contact your database administrator and ask the for the following query to be performed on your patient database:
"Create a list of all my patients according to the following order and conditions:

Group I- all patients with the folowing diagnoses on their problem list:
(ISCHEMIC HEART DISEASE OR CEREBROVASCULAR DISEASE OR PERIPHERAL ARTERY DISEASE)*
    
Group II- all patients with the folowing diagnosis on their problem list: (DIABETES)*   EXCLUDING patients in group I
 
Group III- all patients with the folowing diagnosis on their problem list:
(HYPERTENSION)* EXCLUDING patients in groups I AND  II
 
Group IV- all patients with the folowing diagnosis on their problem list:
(DYSLIPIDEMIA)* EXCLUDING patients in groups I, II and III
 
Group V- all patients with the folowing diagnosis on their problem list:
(SMOKER)* EXCLUDING patients in groups I, II, III and IV.

* Include all synonyms that are used in your database for the different diagnoses- e.g. ischemic heart disease= coronary artery disease= coronary heart disease = angina pectoris=myocardial infarction, etc...).
 
This query will result in a list of all patients in the clinic who have one or more cardiovascular risk factor/s.

3. Go over each of these patient’s files, initiate the monitoring process, and draw operative conclusions. (See Monitoring Protocol).
4. Schedule the next monitoring cycle. (See Monitoring Intervals).
